CC3N are pleased to publish our Rehabilitation Handover document which has been developed through a multi professional sub-group of experienced clinical staff and is endorsed by the Intensive Care Society and the Critical Care Networks. This document has been developed to augment medical and nursing handover on discharge from Critical Care and address compliance with NICE Quality Standard 158 No. 2 for the ultimate benefit to patients' on-going care and experience
